A MESSAGE FROM MYRLAINA
Welcome to the Senior Self-Management Education Project. Led by Myrlaina Corbett, BSN, RN, DNP Student, this initiative reflects her servant's heart and her distinguished 20 plus year career in healthcare. Most of that time has been spent dedicated to helping seniors lead better lives. She deeply appreciates you taking the time to participate in her quality improvement project to empower older adults in managing chronic health conditions.
HOW THIS PROJECT WORKS
- Step 1: Complete the Pre Survey. Provide your Participant ID and complete this first to help us measure confidence levels.
- Step 2: Review Heart Health. Review materials and complete the Follow Up Survey within one week.
- Step 3: Review Diabetes Education. Follow the same process using your Participant ID.
- Step 4: Review Arthritis Education. Review and complete the final education Survey.
- Step 5: Complete the Post Survey. The final step to understand overall program impact.

Need assistance with the website, educational materials, surveys, or Participant ID? In person support will be available at The HUB Center for Seniors during the following times:

Participants are welcome to stop by during any scheduled assistance hours for help navigating the website, completing surveys, locating educational materials, or answering questions about the project.

- What kind of in-person assistance is available? You can receive one-on-one guidance for completing surveys and navigating educational materials at our local community partners.

- Can I complete everything in one day? Yes, participants can finish all surveys in one day or spread them out.
- Is my information confidential? Yes, no personal health info is collected; responses are for evaluation only.
- Should I read materials before the Pre Survey? No, please complete the Pre Survey first to accurately measure knowledge changes.
